About the Conference

The coordination and continuity of health care for a patient moving from one setting of care to another are major challenges for health care professionals and service providers nowadays. The conference aims to provide an opportunity for local and overseas experts to exchange knowledge and latest findings on the topic. It will include an opening ceremony, keynote and plenary speeches, free paper presentations and poster presentations.
